TO THE PRACTICE WOODSIDE HEALTH CENTRE WEBSITE
Woodside Health Centre is a modern purpose-built health centre which
was opened in December 1995 and provides facilities for three separate
GP practices and Croydon Community Health Trust.

Reception is open from 8.30am to 6.30pm Monday to Friday and 8.30
to 11.00am on Saturdays. All telephone calls are queued so please
keep ringing and your call will be dealt with as quickly as possible.
DISABLED
ACCESS
There is disabled access to all areas of the health centre including
surgeries and toilets - please notify the receptionists, and/or
member of staff being consulted, if you have any hearing or sight
impairments and require help.
REGISTERATION
It is the policy within Woodside to accept new registrations according
to an 'on-take' rota, which ensures that there is only one practice
at a time taking new registrations. However, if a patient wishes
to register with a particular practice or is living with someone
already registered, please discuss this with the receptionist. There
is also a shared rota to cover Saturday mornings which may mean
that patients are not seen by their usual doctor for emergency appointments.
LANGUAGES
We encourage patients to bring a friend or relative to translate
if there are language problems.
OUT
OF HOURS EMERGENCIES - Telephone (020) 8655 1223
There is a service for urgent out of hours emergencies which is
contacted by the above number. You will be asked for details of
the problem and either seen or given advice and/or referred to the
surgery the following morning.
This
service is for genuine emergencies only and a detailed list of patients
requesting advice and treatment is forwarded to the practice each
day.
ACCESS
TO MEDICAL RECORDS
You have the right to see your health records kept since 1st November
1991, subject to any limitations in the law. There is a fee payable
for this; all requests must be received in writing and an appointment
will then be offered. We are registered in accordance with the Data
Protection Act.
